Returning Allergy & Injectable Medications Patients

Welcome back! If you are an established allergy or injectable medication patient, you can check out your serum whenever you are going home to your allergist and check it back into us when you return. Please remember to drop off your allergy or medication serum at least 2 days prior to your appointment so we have time to process any necessary paperwork.

1.    Your allergist or physician may mail us the serum or you may bring it in during serum drop off times

2.    For returning allergy patients: once each year we require a copy of the “Allergy History Information Sheet” be completed by you and your allergist.

For returning injectable medications patients
: once each year we require a copy of the "Injectable Medications Questionnaire" be completed by you and your physician.

The form may be sent with the serum or faxed to (517) 432-9460. Any significant diagnoses or history of reactions should be noted on that form.

3.  The injection and dosing schedule (shot record) that comes with the serum should detail the following:
  1. Two patient identifiers (name and date of birth) on all paperwork and vials
  2. Vials labeled with specific contents, dilutions, and expiration dates
  3. Dosing schedule, including adjustments for late/missed dose
  4. Instructions for treatment of local reaction
  5. Instructions for dosing/schedule adjustment after a local reaction
4.    Please be advised that all injection and dosing schedules must be signed by your allergist for allergy injections, or by your physician for injectable medications. We cannot accept orders signed by any other staff members. Also, note that each antigen serum vial and paperwork must be clearly labeled with:
  • Patient name
  • Patient date of birth
  • Contents of serum
  • Dilution
  • Expiration dates of serums

5.    If the documentation is not accurate and complete, you will need to return the vials to your allergist or physician for correction.

6.    When we have received the serum and complete documentation, we will call you to set up your appointment.

7.    We ask that you arrive at least ten minutes prior to your appointment.  If you arrive after the appointment has started, you may be asked to reschedule.
 
8.    For your safety, you will be expected to remain in the clinic for 30 minutes after you receive your injection. If you cannot remain in our clinic for that amount of time, you will not receive your injection.
